Community Partners – Two Harbors Living at Home/Block Nurse Program’s (CP) mission is to support the independence of people 55+ and their caregivers in the Two Harbors area. Our mission is accomplished by organizing volunteers and providing support services, information, and coordination of health and social services for older adults and their family caregivers so that they may live well, independ
ently, and safely in their community and own homes for as long as possible. We want to see all older adults in our community age-in-place and age successfully! We serve the City of Two Harbors, the 21-mile stretch along the North shore of Lake Superior from Knife River to Gooseberry River, and the area up to 5 miles inland from Two Harbors. CP is a member of the Living at Home Network, and was established by community professionals and volunteers in March of 2000. Our Services:

In addition, we coordinate volunteers for Meals on Wheels deliveries in collaboration with Lake View Hospital and Arrowhead Economic Opportunity Agency (AEOA). We ask for a cost-share donation for all of our services based on the participants’ income (excluding participants on waiver); however, we do not want fees to be a barrier to accessing our services and still provide services regardless of one’s inability or unwillingness to pay. Leadership and Funding:
A twelve-member local board of directors provides leadership for our organization. We are funded by a variety of local, state and federal sources including: State of MN, Arrowhead AAA Older American’s Act Programs, Lake County, Northland Foundation, United Way, Two Harbors Area Fund (part of Duluth Superior Area Foundation), CL&P, and many generous local individuals and businesses. Donations, memorials, and honorariums are greatly appreciated.
