Aloha friends and family WHO LIVE IN HAWAII. John A School of Medicine is working with the Hawaiʻi Department of Health (DOH) on the vaccination plan and would like to ask for your support of their efforts to better understand the public’s perception and understanding of the new vaccine. They have commissioned Olomana Loomis to conduct a survey to assess vaccine awareness, receptiveness, and concerns in our community. This survey is a local initiative that is part of the national emergency response to address the pandemic. All information collected is confidential and restricted for use by the DOH to develop and plan the communications of the COVID-19 vaccination program.
The diverse interests and cultures that make up our state require us to listen to and understand many voices. Your feedback on this survey is vital to guiding DOH's COVID-19 vaccination efforts to serve our community and better reach all the people of Hawaiʻi.
Please complete the survey as soon as you can, and no later than Sunday, Dec 13.

The results will be used to inform decisions regarding the placement of resources to address identified needs across the State.
