Community Voices: Opioid Use Awareness Survey
As part of The Confederated Tribes of Warm Springs (CTWS) public health efforts, we are gathering community input to better understand local views on opioid (such as fentanyl) use, which will help guide prevention and treatment planning. This survey is intended to be completed by Warm Springs Tribal members and community members over 18 years of age.
There are 13 questions in this short survey, and it will take about 4-5 minutes to complete.
Participation is voluntary: you may skip any question or stop at any time.
There are no right or wrong answers—we want your honest perspective.
Your responses are anonymous and will go straight to our evaluation partner, NPC Research. We will not collect your name or any other identifying information about you.
In the reports, NPC Research will summarize survey responses like this: “20% of respondents said that opioid use is a serious problem in our community.”
