The individuals and families that we serve are at high risk for a number of poor health outcomes due to several compounding factors, including: poverty, social isolation, lack
of transportation, lack of health insurance, low levels of education, cultural and linguistic barriers, poor diet, little knowledge of risk of substance abuse, breast cancer, HIV/AIDS, birth defects, and a lack of familiarit

y with and trust of the U.S. health care system. Our Community Engagement programs empower community members to increase participation in civic and community activities through education, increased awareness, and outreach activities. These programs allow individuals and families to make informed decisions and take action to positively change their community. Our programs are based on a community-centered model of increasing the awareness and knowledge of community members of key health and wellness issues so that they can access the resources they need for themselves and their families.
