Oasis Community Care We offer both Targeted Case Management and Mental Health Counseling to Medicaid recipients within our community.

We provide the resources and tools to help empower you to improve your life. Our services are conducted in-home allowing you to receive the help you need in the most comfortable and convenient setting for you. Our goal and mission is:

•We connect clients to community resources.
•Assistance in schedulin

•Help find and secure stable housing. Assistance in applying for and renewing SNAP and Medicaid benefits.
•Advocate for clients.
•Connect Clients to community resources to help clients care and maintain stability and independence.
