Home Horizon Transitional Support Program

Home Horizon Transitional Support Program We are Home Horizon and the Barbara Weider House, working with youth at risk of homelessness in the Southern Georgian Bay Area.

Home Horizon creates a community where youth who are at risk of or experiencing homelessness are supported and empowered with the resources and skills to transition into adulthood. While you don’t typically see it, homelessness is a real issue in our area. We currently have 27 youth waiting for help from Home Horizon. Swipe left to see some of our results. The first step in dealing with youth faci

ng homelessness is to help them find a safe roof over their head. This could be the Barbara Weider House or to help connect them with other housing service organizations in our community. The first priority is they need to be safe and inside for the night. In addition, we offer access to counselling and social and life skills programs to help our youth successfully rebuild their lives and transition to independent living and adulthood. They are provided with a range of support programs that will help them learn life skills, have positive relationships with peers and adults, and re-engage with school, employment training, and/or employment. The Barbara Weider House bridges the critical gap between emergency shelter and independent living for youth aged 16-24. Our services address the needs of youth in crisis and provide the support to help them master the tasks of adolescence as well as focus on their education and employment possibilities. The pandemic has non-profit organizations cancelling fundraising events and scrambling for new ways to raise funds... and we are no different. Our revenue shortfalls has our fundraising committee working hard to find new and creative ways to engage our community and to ignite people’s interest in supporting our youth. 💙 7 DAYS TO GO! 💙

As we count down to on August 12, we're reminded why Home Horizon exists.

"No one gave me a chance. No one would listen to me, and no one gave a 'crap' until I got here. I have a real chance of being a success now. Thank you so much from the bottom of my heart."

These powerful words came from a young person whose life was changed because someone cared.

Every young person deserves to be heard. To feel safe. To know they matter. To have the opportunity to build a brighter future.

💙 Join us on August 12 as our community comes together to raise awareness and support youth experiencing homelessness.

Every donation. Every volunteer. Every fundraiser ticket. Every act of kindness tells a young person:
You matter. You belong. There is hope.

Together, we can ensure no young person feels they have to face homelessness alone.

Hope grows when a community chooses to show up. 💙

Every day, Home Horizon’s Youth Workers help young people build confidence, gain life skills, reconnect with education, find employment, and take brave steps toward independence. This summer, we’re celebrating three youth moving into homes of their own.

🌈 Everyone Belongs Here. 🌈

This Pride Weekend, Home Horizon proudly celebrates and stands with the 2SLGBTQI+ community.

Every young person deserves to feel safe, respected, and empowered to live authentically. At Home Horizon, we are committed to bridging the critical gap between emergency shelter and independent living by creating a welcoming, inclusive environment where every youth feels seen, valued, and supported.

We believe that building inclusive communities means more than acceptance—it means celebrating diversity, standing against discrimination, and ensuring all young people have the opportunity to thrive. When youth know they belong, they are better able to build confidence, well-being, and brighter futures.

As Collingwood celebrates Pride, July 10–12, we extend our support to the entire 2SLGBTQI+ community, our allies, families, friends, and everyone working to build a more inclusive community.
❤️🧡💛💚💙💜

Have a safe, joyful, and wonderful Pride Weekend.
Love Wins. 🏳️‍🌈
