Yakap Registration: Philhealth ID or MDR
General Primary Requirements (First-Time Registrants)
1. PhilHealth Member Registration Form (PMRF).
2. Valid Government-Issued ID.
3.Birth Certificate.
Requirements by Membership Category
1. Senior Citizens (60 Years Old and Above)
-OSCA ID / Valid Government ID proving age (60+).
- Brgy. Indigency
2. Persons with Disability (PWD)
-Valid PWD ID issued by the Persons with Disability Affairs Office (PDAO) or DSWD.
Valid Government ID.
-Brgy Indigency
👨👩👧 Requirements for Adding Dependents
If you are adding qualified dependents to your record, you must present proof of relationship:
Legal Spouse: PSA Marriage Certificate.
Children (Below 21 years old, unmarried, and unemployed):
PSA Birth Certificate.
Children with Disability (21 years old and above, permanently disabled): PSA Birth Certificate and a Medical Certificate detailing the condition.
Parents (60 years old and above): Member’s PSA Birth Certificate showing parent’s names, plus the parent’s Valid ID or Senior Citizen ID.
