We provide NDIS & Aged Care Support Services including Supported Independent Living, Support at Home Program, Respite, Support Coordination, Plan Management, Ground Control, Social Support, Clinical Nursing, and Daily Living assistance. Services We Offer:
•All carers for people aged 0-64 with a disability outlined in the Disability Service Act are eligible to apply for respite care hours.

•To assist our Customers to plan their personal futures and set goals to achieve those futures.
•To facilitate development of Customers skills and abilities to enable them to look after themselves in the accommodation they have
chosen.
•To provide opportunities for Customers to develop skills so that they are able to take part in the community as equal members.
•To match individual Customers with quality support personnel who can facilitate him/her to achieve the above objectives. Matching
support staff to the person will be based on the individual's wishes and needs.
•To develop a positive image of both the agency and participating individuals in the view of the local and larger community. Brokerage Services:
•Other services may broker support from Gladstone Community Linking Agency and likewise G.C.L.A can broker support
for people from other services.
•Some members of the community may choose to purchase the service out of their own personal funds. Individual Funding Packages:
•People in receipt of an individual funding package can approach GCLA to manage their funds/and provide
staff to suit their own individual needs.
•The individual/family choose the Gladstone Community Linking Agency as the service that they wish to administer their package.
•It is the families/individuals themselves that point us in the direction that they wish to go. set goals.
•Gladstone Community Linking Agency develops the individual/family service delivery in accordance with the Principles and Objectives
of the Disability Services Act 2006.
•Gladstone Community Linking Agency staff determines the necessity for specific staff training including Workplace Health and Safety
and Duty of Care boundaries and develops relevant individual procedures around the identified needs of the individual/family.
•The Agency matches individual service users with trained support workers who can facilitate him/her to achieve the above objectives. Matching will be based on the individual's wishes and needs.
•To provide individual support to develop independent living skills for young people leaving school.
•Support hours are regularly reviewed with families/individual. A place where differences are valued:
•The HUB is located at 5 Buller St Gladstone, it is where skills are learnt, friendships made by engaging people in meaningful
experiences surrounding differences.
•The Hub is a center available for GCLA customers to use and attend for different programs.
•Learning and creative experiences are available for everyone who wishes to participate.
•We hope to enrich the community more by ensuring everyone is valued, everyone is embraced and everyone has a sense of belonging, and
difference is a natural part of life.

GCLA is seeking a Senior Support Worker to join our High Intensity Supported Independent Living (SIL) service in Gladstone.

This is an opportunity to step into a leadership role, providing on-the-floor guidance, mentoring and support to team members while helping deliver safe, consistent and person-centred support to customers with complex needs.

In this role, you’ll:
✨ Lead shifts and support team members
✨ Provide high-quality, person-centred support
✨ Support customers with complex and high-intensity needs
✨ Assist with daily operations within the SIL home
✨ Support decision-making and respond to complex situations
✨ Promote a positive and supportive team culture

We’re looking for someone with:

💙 What an incredible day at Port to Park! 🏃‍♀️🏃‍♂️

It was an absolute honour for Gladstone Community Linking Agency (GCLA) to be the community beneficiary of this year's Gladstone Ports Corporation Port to Park Fun Run.

We would like to express our sincere gratitude to Gladstone Ports Corporation for choosing GCLA and for their incredible support of our organisation and the people we support.

The funds raised will go towards the purchase of a new wheelchair-accessible bus, which will help us provide safer, more reliable and accessible transport for our NDIS and Aged Care customers.

This support will make a real difference, helping the people we support get out into the community, participate in activities and stay connected with the people and places that matter to them.

On behalf of our NDIS and Aged Care customers, and our staff, thank you. 💙
