PASSPORTS AVAILABLE FOR RELEASE:
Releasing Time:

Monday to Friday
9:00 am – 11:00 am
2:00 pm – 4:00 pm

Please bring the following:
- Official receipt; and
- Renewed passport

For applicants who cannot claim their new passport themselves, the
following are the requirements:

A. Claiming of Passports for immediate family members:
a. Authorization letter (must mention affiliation with applicant);
b. Original receipt and old passport; and
c. Valid ID (original and photocopy) of applicant and representative.
* Immediate family members of legal age such as parents, brother, sister,
spouse or children

B. Claiming of Passports for non-immediate family members:
a. Authorization letter notarized/legalized by Mukata’a;
b. Original receipt and old passport; and
c. Valid ID (original and photocopy) of applicant and representative
indicated in the Authorization letter.

C. Claiming of Passports for minor applicants:
a. If passport will be claimed by the mother, claimant must present
original receipt, old passport and her valid ID with photocopy.
b. If the passport will be claimed by the father, the claimant must
present the original receipt, old passport and his valid ID with
photocopy and PSA documents indicating proof of parental authority
over the minor.

Passport will not be released if these requirements are not complied with.
