🧓 Proof of Birthdate for 60 or older (if applicable)
♿ Proof of Disability (if applicable)
💰 All Household income received Daily, Weekly, Bi-weekly, and/or Monthly must cover the date range of 5/13/2026 - 6/12/2026
🍅 Proof of Food Stamps for Current Month (if applicable)
💡 Two (2) most recent itemized utility bills (all pages)
🏘️ Section 8 or HUD Housing Shelter form for Current Month (if applicable)
