The Logan County QRT works together to ensure no one falls
through the cracks when it comes to substance use and
recovery support.

We’re excited to share a new public-facing referral link that makes it easier than ever to connect someone to help. This tool allows:
✔️ Community members
✔️ Family and friends
✔️ Employers or coworkers
✔️ Self-referrals
to request a QRT follow-up for someone who may be struggling with substance use—or if you’re unsure where to start but know support is needed.

This referral form is available 24/7/365, does not require a login, and works on any device.
