Please Bring: Current State Issued ID, Social Security Cards for All Adult Household Members, All Household Income (All Paycheck Stubs for the month of February, Social Security Award Letter, etc.) Proof of Child Support if Children are in the home, If Unemployed must provide unemployment statement and other zero income documentation, Copy of Lease Agreement for Subsidized Housing and Utility Bills (Electric and Gas).
