The Genesee County Career Center will temporarily close for in-person appointments and walk-in services November 3–27, 2026, as part of the New York State Department of Labor’s unemployment insurance system upgrade. Reopening is scheduled for Monday, November 30, subject to change.

Career assistance will remain available virtually and by phone, including help with resumes, interviews, job searches, and career planning.

We are seeking a dedicated Registered Nurse Care Coordinator to join our HomeCare & Hospice team. This role combines clinical excellence, leadership, and meaningful patient connection while supporting individuals and families in the comfort of their homes.
Position Overview
Reporting to the Senior RN Care Coordinator, the Registered Nurse Care Coordinator is responsible for:
• Coordinating patient case management
• Supervising and evaluating paraprofessional staff
• Conducting patient assessments and updating plans of care
• Providing direct skilled nursing services
• Collaborating with the Interdisciplinary Team (IDT)
• Participating in on-call rotation (1 weekday during the week and 1 weekend a month)
• Ensuring compliance with agency policies and standards
Qualifications
Current NYS Registered Nurse License
Minimum 2 years recent nursing experience (preferred)
BLS Certification
Valid driver’s license and access to an insured vehicle
EMR proficiency
Strong written and verbal communication skills
