Our presenter, Hazel Miller, is an Options Counselor Supervisor for the Aging and Disability Resource Center (ADRC) of Milwaukee County. Hazel and her staff assist individuals to plan for short-term and long-term needs, access community services and resources, and learn about programs and benefits available.

What is the Children's Long-Term Support (CLTS) Waiver Program?

The Childrenā€™s Long-Term Support (CLTS) Waiver Program is a Medicaid Home and Community-Based Waiver program that provides services for children with disabilities.

The program is administered in Milwaukee County by the Milwaukee County Department of Health & Human Servicesā€™ Children, Youth, & Family Services (CYFS).

WHO IS ELIGIBLE FOR CHILDRENā€™S COMMUNITY OPTIONS PROGRAM (CCOP)?

Children (ages birth through 21) who live in Milwaukee County
Must have a physical, emotional, or developmental condition
All diagnoses must be current and made by an appropriate medical or mental health professional. Mental health diagnoses require additional documentation such as a psychological evaluation or other norm-referenced testing

In addition, the childā€™s condition is likely to result in substantial limitations in their ability to function in three or more of the following areas:
ā€¢ Self-care
ā€¢ Receptive and expressive language
ā€¢ Learning
ā€¢ Mobility
ā€¢ Self-direction
ā€¢ Capacity for independent living
ā€¢ Economic self-sufficiency
