Organized in 1999, CHW has grown into a multi-county collaborative that seeks to improve the health of local residents and the capacity of the local health care community. The original core service area of CHW includes Bibb, Crawford, Houston, Jones, Monroe, Peach and Twiggs Counties. Over the past decade we have expanded our reach to more than 1/3 of the state, including, but not limited to publi
c health district 5-1 and 5-2. Our four main areas of focus include: improving access to care, improving health literacy among youth and adults, improving the quality and coordination of health care providers and health care systems, and improving coordination and regional integration of health information technology. Our clients and partners include rural central Georgians and the community at-large, regional health care providers and networks, community leaders, and policymakers. At present, the main health issues we seek to address and/or eliminate through our various programs include obesity, nutrition and wellness, diabetes, cardiovascular disease, stroke and cancer.
