COMMUNITY CONNECTIONS

This program provides highly individualized
crisis intervention, advocacy and navigation
services to qualifying individuals. This program
aims to connect clients with community
support systems that meet their individual
needs. Services include but are not limited to:
• Housing assistance
• Connecting to food support programs
• Assistance in applying for publicly
funded progr

• Shopping, budgeting, financial and
application assistance
• Home visiting and home management
skills
• Safety planning
• Access to medical support and followup
on medical directives
• Employment and volunteer readiness
and placement
• Support with legal issues, daycare,
school and landlord/tenant issues
• Ongoing one-on-one daily living skills
counseling and general support
Staff work directly with participants to identify
and resolve barriers and typically accompany
participants on initial meetings and appointments to help facilitating connections. THERAPEUTIC RECREATION

This program provides skill development
classes and small and large group wellness
outings that reinforce physical activity, healthy
life styles and behaviors to improve health
outcomes and raise awareness of community
resources that promote wellness. Each month,
approximately 20 daytime, evening and weekend low-cost activities are offered. Transportation is provided from Southside Services to activities and to participants’ homes. A monthly Therapeutic Recreation Activity Calendar (and agency newsletter) is mailed to all Southside Services participants who must register for each activity by calling the Program Coordinator. A wide variety of activities are offered including:
• Camping trips
• Cultural arts venues
• Sporting events
• Dances
• Day trips
• Competitive sports leagues (bowling,
softball, basketball, touch football,
kickball, bocce ball, etc.)
• Fitness centers
• Seminars and workshops
• Fairs
• Healthy eating cooking classes
• Neighborhood festivals
