Q&A: Shoulder pain is the 𝘁𝗵𝗶𝗿𝗱 𝗺𝗼𝘀𝘁 𝗰𝗼𝗺𝗺𝗼𝗻 𝗿𝗲𝗴𝗶𝗼𝗻 for which individuals seek care, and 𝗺𝗼𝗿𝗲 𝘁𝗵𝗮𝗻 𝗵𝗮𝗹f of treated shoulder pain cases are 𝗿𝗼𝘁𝗮𝘁𝗼𝗿 𝗰𝘂𝗳𝗳 𝘀𝘆𝗻𝗱𝗿𝗼𝗺𝗲𝘀. 💪

A 21-year-old college volleyball player develops subacute shoulder pain and is diagnosed with a rotator cuff tear. Rather than defaulting to a generic protocol, this case report applies a new multidisciplinary Rotator Cuff Tendinopathy Clinical Practice Guideline (CPG), prioritizing motor control and resistance training, with manual therapy to manage pain.

In just 𝟳 𝘃𝗶𝘀𝗶𝘁𝘀, he saw 𝗰𝗹𝗶𝗻𝗶𝗰𝗮𝗹𝗹𝘆 𝘀𝗶𝗴𝗻𝗶𝗳𝗶𝗰𝗮𝗻𝘁 𝗴𝗮𝗶𝗻𝘀 in strength symmetry, function, and patient-reported outcomes. The case also surfaces where the new guideline still needs refinement, from diagnostic criteria to loading parameters.

A great read for any PT curious about turning a new CPG into real clinical decision-making.

Did you know? Peroneal nerve palsy is the 𝗺𝗼𝘀𝘁 𝗰𝗼𝗺𝗺𝗼𝗻 𝗹𝗼𝘄𝗲𝗿 𝗲𝘅𝘁𝗿𝗲𝗺𝗶𝘁𝘆 𝗽𝗮𝗹𝘀𝘆, and trauma to the posterolateral corner (PLC) of the knee with multiligamentous injury leads to it in as many as 𝟰𝟰% 𝗼𝗳 𝗰𝗮𝘀𝗲𝘀. 🦶

An 18-year-old athlete hyperextends his knee. No contact, no dramatic fall, and days later, 𝗵𝗲 𝗰𝗮𝗻'𝘁 𝗹𝗶𝗳𝘁 𝗵𝗶𝘀 𝗼𝘄𝗻 𝗳𝗼𝗼𝘁. This case report from the July issue of 𝘖𝘗𝘛𝘗 follows his 12-session course of physical therapy targeting strength and sensory recovery, tracking the gradual return of sensation from the fibular head to the great toe, along with trace activation of the tibialis anterior.

The clinical takeaway: 𝗽𝗼𝘀𝘁𝘁𝗿𝗮𝘂𝗺𝗮𝘁𝗶𝗰 𝗸𝗻𝗲𝗲 𝗮𝘀𝘀𝗲𝘀𝘀𝗺𝗲𝗻𝘁𝘀 𝘀𝗵𝗼𝘂𝗹𝗱 𝗮𝗹𝘄𝗮𝘆𝘀 𝗶𝗻𝗰𝗹𝘂𝗱𝗲 𝗮 𝘁𝗵𝗼𝗿𝗼𝘂𝗴𝗵 𝗻𝗲𝘂𝗿𝗼𝗹𝗼𝗴𝗶𝗰𝗮𝗹 𝗲𝘅𝗮𝗺, not just structural testing. Missing early signs of PLC involvement can mean missing the window to prevent lasting nerve dysfunction.
